• The 1-hour consultation is not a membership plan. This entitles you
  to a one-time, one-hour-long visit only. Any promotional discounts and cash payment discounts are not applicable. If you decide to avail yourself of any of the membership plans within 7 days of the visit, your payment will be applied to your membership fee.
   

• Lifestyle Medicine and Wellness Practice PLLC will contact you within 24 hours from receipt of your enrollment form to arrange payment, schedule an appointment, and obtain necessary information. Your membership plan will start on the date of your payment.
